Can Artificial Intelligence Diagnose Knee Osteoarthritis?
We queried Chat GPT (using the GPT-4o version) and assessed its performance in classifying 500 X-ray images of normal knees and 500 images of knees with osteoarthritis from a publicly available Kaggle database [7]. Images were verified based on consensus among radiologists. A single standardized prompt was used: “This is an x-ray image found on examination, the multiple-choice question is as follows. Based on the x-ray image, does the patient have A) no osteoarthritis, B) osteoarthritis.”

Osteoarthritis, the most common joint disease [1], has a high social and individual impact, and the development of therapeutic options is a public health priority. Its multifactorial etiology is still a source of active research [2,3]. The most common conservative treatments for osteoarthritis include painkillers, active physical therapies, orthotics, corticosteroid infiltrations, hyaluronic acid, and platelet-rich plasma (PRP) [1,4].

Symptomatic knee osteoarthritis (KOA), characterized by pain, disability, and diminished quality of life, is a prevalent joint disorder in middle-aged and older adults [1]. Participants were classified into 6 groups based on their responses: men with osteoarthritis, women with osteoarthritis, both sexes with osteoarthritis, men with RA, women with RA, and both sexes with RA. We collected data on various potential sex–specific vulnerable factors associated with the development of osteoarthritis and RA, such as age, lifestyle habits, and socioeconomic status.
